Four-step process

How to refer

Referral process (4 steps):

  1. 01Complete the referral – email a referral letter or summary including presenting concerns, relevant history and reason for referral.
  2. 02Submit securely – send by email or post. Do not include identifiable patient data by unsecured email without consent.
  3. 03Acknowledgement – we acknowledge all referrals within 2 working days and will contact you or the patient directly to confirm next steps.
  4. 04Assessment & feedback – following assessment, a clinical report or feedback letter is sent to the referrer (with patient consent).
